After scheduling your first appointment, we will mail you an welcome information package. It will include a medical history form, which we ask you to complete and bring to your first appointment. You can find the new patient forms below online, print them and complete at your convenience. During your initial visit to our office, you will be given a preliminary clinical exam and diagnosis. This may include complementary diagnostic records such as digital photographs and radiographs. Dr Dann will also use computer simulation and case examples to help in your dignosis and treatment planning. At this time all questions will be answered for you. Some common questions are:
- Do I need treatment?
- When do I need treatment?
- Approximately how long will treatment take?
- What type of appliances or braces are appropriate for me?
- What will the cost be?
If orthodontic treatment is indicated futher comprehensive diagnostic records may be scheduled.

Diagnostic Records
Dr Dann may recommend that diagnostic records to be taken. At this appointment, impressions for study models, facial / oral photographs, and radiographs may be necessary for diagnosis and treatment planning. Dr. Dann will study these records to determine what problems are present and what treatment options are recommended.
Consultation Appointment
At the consultation, findings from the diagnostic records, specific orthodontic problems and detailed treatment plan(s) are presented. The consultation is often done at the initial visit appointment. Anticipated treatment time, financial arrangements and patient responsibilities will be discussed. The fee for your treatment and our convenient payment plan will be discussed. We welcome patients with dental insurance, and we will make every effort to see that you receive your maximum benefits. Also, at this time, appointments will be made to start treatment.
