First Visit/Patient Forms

After scheduling your first appointment, we will mail you an information package. It will include a complete medical history form, which we ask you to complete and bring to your first appointment. During your initial visit to our office, you will be given a preliminary clinical exam and diagnosis. At this time all questions will be answered for you. Some common questions are:

  • Do I need treatment?
  • When do I need treatment?
  • Approximately how long will treatment take?
  • What type of appliances or braces are appropriate for me?
  • What will the cost be?

If you need treatment, an appointment for comprehensive diagnostic records will be scheduled.

For your convenience, you may fill out the form online and print it out to bring with you on your visit to our practice.

Diagnostic Records

If treatment is indicated, diagnostic records will need to be taken. At this appointment, impressions for study models, facial and oral photographs, and orthodontic X-rays are taken. Dr. Dann will study these records to determine what problems are present and what treatment options are recommended.

Consultation Appointment

At the consultation, findings from the diagnostic records, specific orthodontic problems and detailed treatment plan(s) are presented. Anticipated treatment time, financial arrangements and patient responsibilities will be discussed. The fee for your treatment and our convenient payment plan will be discussed. We welcome patients with dental insurance, and we will make every effort to see that you receive your maximum benefits. Also, at this time, appointments will be made to start treatment.
